REQUIRED CAMP FORMS
CAMPERS ARE REQUIRED TO HAVE THE FOLLOWING FORMS UPLOADED TO THEIR SAWYER PROFILE BEFORE THEY MAY ATTEND CAMP
-REGISTATION AND POLICY PACKET
-liability waiver
-sunscreen authorization form
-copy of immunization records or exemption form
these forms may be found here on our website (SAME FORMS AS SUMMER CAMP FORMS) or they can be downloaded during registration. parent/guardians will fill out each camper's form and then upload it to their sawyer profile. instructions can be found below.
